Patient appointments are scheduled from 8:30-4:00pm Monday through Friday.

We are closed for lunch each day from 12:00pm to 1:00pm.

• If you are unable to keep your appointment and must cancel or reschedule, please contact us so that we may schedule that time for another patient. We kindly ask for 24 hours notice of any cancellation whenever possible.

• Co-Pays and deductibles are due at the time of your appointment.

• We ask you to arrive 10 to 15 minutes prior to your appointment so that your records may be updated.

• Lab results are returned directly to our office. Once we have analyzed the test results, we will contact you.

• Prescription/Refill requests for pain medications, antibiotics, and any controlled substance REQUIRE an office visit.

• Any work-related visit requires a Workman’s Compensation Authorization be provided at the time of the appointment.

• Please bring any current prescription and/or over-the-counter medications, vitamins, or supplements with you to your appointment.

If you believe your situation is a MEDICAL EMERGENCY, CALL 911 or go immediately to the nearest HOSPITAL EMERGENCY ROOM for treatment. We ask that you contact us as soon as possible thereafter so that we may follow -up and assist in your care.
